Healed is a consumer brand of Sigma Health, Inc. Sigma provides non-clinical membership services and does not practice medicine. Medical services are provided by independent licensed healthcare professionals through professional entities supported by OpenLoop Health, Inc. OpenLoop operates the Healed-branded patient platform and coordinates patient billing, clinical operations, pharmacy, laboratory, and fulfillment services.
2. Treatment and telehealth disclosure
Telehealth is not appropriate for every person or condition. A licensed treating professional may require additional information, laboratory testing, a telephone or video consultation, an in-person examination, or referral elsewhere. Completing an assessment or purchasing a membership does not guarantee eligibility, a prescription, medication, or any particular result.
Patients must review and accept the OpenLoop Telehealth Consent and the other policies presented in the OpenLoop-operated patient platform.
3. Compounded-medication disclosure
If a licensed treating professional determines that a compounded medication is appropriate, understand that compounded drugs are not FDA-approved. The FDA does not review compounded drugs for safety, effectiveness, or quality before they are marketed. Compounded medications are not represented as generic versions of, equivalent to, or the same as FDA-approved drugs. Availability may change.
4. Emergency disclosure
This service is not for emergencies. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
5. Privacy and medical-record disclosure
The treating professional’s medical record is maintained in the OpenLoop-operated system. The applicable OpenLoop Notice of Privacy Practices explains how protected health information may be used or disclosed and how to exercise medical-record and HIPAA rights.
